Hello and welcome! My name is Danielle Donohue, BSN, RN. I’m a Registered Nurse Care Manager in the State of Maine. I created Evergreen Care Management in an effort to provide a compassionate, evidence-based, and reliable resource to families across the state who are navigating the difficulties of remaining in their home while managing acute and chronic illnesses. Evergreen is a private pay service which allows more control over creating customizable care plans without the burden of insurance company oversight or the need for prior authorizations. Evergreen is not a home healthcare “agency”. These services can be provided in the home, community living facilities, assisted living facilities, hotel (if having traveled and are recovering from a surgical procedure) - wherever you are recovering. I keep my client base small, ensuring tailored, personalized care. One ER visit can easily cost a family thousands of dollars out of pocket. It is my goal to catch small changes before they turn into an emergency room visit, and I am invested in keeping your loved one at home and out of the hospital.
